CooperativeCare offers the excellent health care employees deserve at a local level. With 100% of the physicians (MDs and DOs) in the CooperativeCare service area participating, employees receive quality care and the advantages they want most in their health care plan, including access to local providers and care. Through the combined resources of network hospitals, convenient and accessible care is easily available. In addition to traditional inpatient care, advanced techniques such as laser and endoscopic procedures and heart catheterization are available on an outpatient basis.
CooperativeCare has a complete and comprehensive local network. Services available through CooperativeCare vary from pre-testing, laboratory, X-ray, diagnostic and therapeutic procedures to well-child visits and transplants. You can be confident that complete care – from health education and wellness training to catastrophic care – is available through CooperativeCare.
The CooperativeCare network is comprised of a wide variety of primary and specialty care physicians. Whether you need a family doctor to take care of the common cold, an orthopedic surgeon for a knee replacement or an oncologist for aggressive cancer therapy, the members of the CooperativeCare Regional Provider Network are available and qualified to take care of you.
Our Provider Search directory lists doctors, hospitals, other providers and services within the CooperativeCare network. You are eligible for maximum coverage according to the terms of your health care plan when you receive services from any of the providers listed in this directory.
If you are referred for services not available within the local primary network, please request a referral to a CooperativeCare tertiary provider from your local physician or contact CooperativeCare directly. (A tertiary provider is a hospital or medical center that presents highly specialized services not offered within the local network.)
If you have questions about a provider, please call (859) 239-2471.
